You can complete this form without registering or signing in.

A carer is anyone who cares, unpaid, for a friend or family member who due to illness, disability, a mental health problem or an addiction, cannot cope without their support. You don’t have to live with the person you care for to be a carer.

You can use this form if you would like our Carer Support Team to offer you advice and information on

  • Your rights as a carer
  • Local services including short breaks and 1:1 support
  • Looking after yourself
  • Hospital admission and discharge
  • Completing an Adult Carer Support Plan
  • Setting goals and priorities for your own life.

This form can also be used to refer someone else for carer support, if they are aware of and have consented to this.

Service for hard of hearing

Service for deaf people

To contact the Council you can use the British Sign Language interpreting service free of charge.  This is accessed  via Contact SCOTLAND-BSL or by using Scotland BSL app
