Working with young people leaving care
We will send you a letter about Throughcare and Aftercare if you are a social worker for a young person in Edinburgh aged 15½ who is either
- on a supervision order
- in foster care, kinship care or a residential unit or school.
We send out letters a few months before the young person's 16th birthday.
Not all young people will need our help at this time, but please complete the forms and return them to us to let us know. You can activate the referral at any time thereafter.

What happens next
Before a young person stops being looked after or accommodated we must complete an assessment and plan. Then there will be a transition meeting before Throughcare and Aftercare becomes the key team for the young person.
We run sessions about our service for professionals every eight weeks. Contact the duty worker to find out when the next one is.
After the young person has been referred, ask the duty worker to arrange a meeting to introduce the young person to us.
