Apply for a Taxicard

In order to apply, you must complete all parts of the online form.
You will need
- Signed eligibility criteria form if applicable. Uploading a pre-approved verification form is the quickest way to apply and get approval as we must contact your doctor. If you have sight loss, you can upload an RNIB certificate of vision impairment.
- Payment card
- Passport style photograph. If you use a smart device, the form will allow you to take a photo of yourself or you can attach a photo from your device. Photos must be original do not take a photo of an existing image.
If you currently receive a service from any of the following agencies ask one of them to complete the eligibility criteria form. No other verification form will be accepted. (Please note that some of our downloads might not be accessible. If you need help please contact us on 200 2000.)
