Taxicard

There is more than one way to apply for a Taxicard and you can do this online or by post.

Apply online

If you apply for a Taxicard online, you will be asked to upload a signed eligibility criteria form. If you cannot provide this, we will need to contact your doctor to ask them to verify your application. Applicants with sight loss can provide an RNIB certificate of vision impairment.

Your eligibility criteria form must be signed by a health or social care professional. Examples of professionals who can sign your form are listed below.

Health and social care professionals

• Social worker
• Home care organiser
• Occupational therapist

Health board

• Occupational therapist
• District / community nurse
• Health vistor
• Royal National Institute for the Blind                                            

Once you have either provided an eligibility criteria form, an RNIB certificate of vision impairment, or have asked us to contact your GP, you will be asked to enter payment card details and will need to upload a passport style photograph. If you use a smart device (such as a mobile phone with a camera), the form will allow you to take a photo of yourself. Photos must be recent and original, and should not have been used on any previous application.
